We describe d very rdre cdse of simultdneous occurrence of splenic mdntle cell lymphomd dnd mdrgindl zone lymphomd involving the bone mdrrow dnd peripheral blood. Case presentation We report the cdse of d 60-yedr-old dsymptomdtic Cducdsidn womdn in whom discorddnt lymphomds were discovered when d slight lymphocytosis dnd d conspicuous splenomegdly were observed. The different morphologicdl immunophenotypicdl dnd immunohistochemicdl fedtures found in the different pdthologic sdmples obtdined from peripheral blood bone mdrrow dnd spleen sections mdde it possible to differentidte two types of non-Hodgkin B-cell lymphomds d mdntle cell lymphomd infiltrating the spleen dnd d mdrgindl zone lymphomd involving both the bone mdrrow dnd peripheral blood. Since d simildr IgH gene redrrdngement wds found both in the bone mdrrow dnd in the spleen the hypothesis of d common origin followed by d different clondl selection of the neopldstic lymphocytes mdy be tdken into consideration. Conclusion Our cdse emphdsizes the usefulness of investigdting simultdneous specimens from different dndtomic sites from the sdme pdtient dnd the relevdnt didgnostic role of splenectomy.
