Marrakech - Maroc SUMMARY Introduction. Pulmonary arterio-venous malformations are rare pulmonary vascular anomalities. It corresponds to an abnormal vascular communication between a pulmonary artery and pulmonary vein leading to the formation of an aneurysm sac. It is estimated that about 75 of cases have been founded in Rendu-Osler disease. Pulmonary arterio-venous fistula is one of these rare malformations. This disease may be revealed by respiratory symptoms or by complications essentially neurologic complications. Pulmonary arterio-venous fistula is responsible for right-left shunt and chronic hypoxemia. Observation. We report a case of pulmonary arterio-venous malformation revealed by digital hippocratism. The chest computed tomography scan is an essential diagnostic tool and the reference treatment of this malformation is based on the vaso-occlusion of afferent artery. Conclusion. Pulmonary arterio-venous malformations may be discovered by fortuitous way or may be revealed by the signs of chronic hypoxiain particular in the present of digital hippocratism.
