This report assess the early outcome of Puestow procedure through 2 clinical cases in our hospital. Case report 2 patients with many recurrent pancreatitis episodes dilated pancreatic duct and pancreatic stones. Preoperative evaluation included blood test liver function tests pancreatic enzymes tumor markers computed tomography magnetic resonance cholangiopancreatography. The dilated pancreatic duct was opened via a longitudinal incision made sharply through the pancreatic parenchyma a lateral pancreaticojejunostomy was created. Postoperatively there was resolution or significant improvement in their abdominal pain symptoms the diameter of pancreatic duct reduced to lt 5mm. There was no episode of recurrent pancreatitis at the time of last follow-up. Conclusions This study reports the intial result of the Puestow procedure for treatment of chronic pancreatitis in children. The advantageous of technique is decompression of dilated pancreatic duct due to obstruction. This technique can be performed for chronic pancreatitis with dilated pancreatic duct gt 5mm.
