We describe the diagnostic workup and management that led to a successful outcome. A 4-month-old female infant presented to our institute with recurrent pneumonia normal obstetric history regular respiratory support. With the assistance of contrast-enhanced chest computed tomography CT upper gastrointestinal tract imaging and upper gastrointestinal endoscopy the diagnosis was established according to the most recent diagnostic criteria. The patient was recieved middle and lower right pneumonectomy and surgical repair of esophageal fistula. CBPFM type III was confirmed on the basis of the postoperative histopathological finding. The drainage was removed 5 days post-op the respiratory problems were improved however the patient still needed to go through a cardiac operation subsequently. Esophageal Bronchus is a rare condition thus the pre-operative diagnosis is extremely difficult clinicians usually focus on lungs parenchymal damaged condition while omit airway abnormalities. Bronchoscopy should be performed in complex cases of esophageal atresia to avoid missing these deformities.
