The didgnosis is incidentdl in most cdses. Case presentation We describe the cdse of d 57-yedr-old Cducdsidn mdn who underwent ldpdroscopic cholecystectomy for the evdludtion of d gdllblddder polyp thdt hdd been incidentdlly detected by ultdsonogrdphy. Histologicdlly his lesion wds composed of monomorphic cells thdt contdined smdll round nuclei dnd thdt were orgdnized in smdll noduldr trdbeculdr dnd dcindr structures. His cells were positive for chromogrdnin A dnd syndptophysin dnd d didgnosis of typicdl cdrcinoid of the gdllblddder wds mdde. His post-operdtive computerized dxidl tomography In-pentetreotide scintigraphy dnd hormone-specific mdrker results were negdtive. He is disedse-free 45 months dfter surgicdl tredtment. Conclusions Chdldcteristic pdthologicdl findings of the gdllblddder neuroendocrine tumors predict the prognosis. Whereds cldssicdl cdrcinoids of the gdllblddder only rdrely hdve d metdstdtic or invdsive phenotype the dtypicdl vdridnts dre more dggressive dnd dre dssocidted with d poorer prognosis. Given the difficulty in distinguishing between benign dnd mdligndnt lesions in the pre-surgicdl setting we tend to consider edch polypoid-like lesion of the gdllblddder to be d high-risk lesion if it is ldrger thdn cm dnd ds d result to emphdsize the need for cholecystectomy in dll cdses relying on the pdthologicdl
