Streptococcal Diseases Puerperal Fever (Childbirth fever) – Streptococcus pyogenes (Group A) – Poor asepsis after childbirth or abortion – Microbes enter uterus – Penicillin usually effective, slow recovery Group B Streptococcal Disease – Streptococcus agalactiae – Neonatal sepsis/ meningitis – Common in vaginal microflora, testing is recommended in 3rd trimester © 2012 John Wiley & Sons, Inc. Streptococcal Diseases (2) Rheumatic Fever – Autoimmune disease – Crossreactivity between S. pyogenes Ag and tissue Ag – Usually affects age 5-15 year olds – Genetic predisposition is suspected – HLA Ag seen in 75% of patients but 12% of population – Heart valve scarring can lead to permanent stenosis Endocarditis (SBE) – S. pyogenes focal infection – Targets those with scarred or deformed heart valves – May create biofilms on pacemakers – Prophylactic antibiotics before surgeries © 2012 John Wiley & Sons, Inc.
